FIRST LEVEL II VALIDATIONS $550.00
Validation Level 1 $550.00
This is an MD validation, with records reviewed and signed by our Physician, including Duties Under Duress and Loss of Enjoyment factors which relate to the causation of the patient’s injury.
Validation Level 2 $800.00
This is an MDV as Level 1, but adds a multipage file of the Impairment Rating Report based on the 6th Edition of the AMA guidelines. This record greatly increases the validity and drives the value of your claim. This Level 2 is the most useful and sought after of all our reports.
Validation Level 3 Starting at $1,500
All the records of Level 2 plus a complete comprehensive review of all medical records for more intricate cases. 1 inch of medical records equals one hour of review.
Validation Level 4 Starting at $2,500
Patient has to be seen by our Doctor. This report includes complex causation, but the price is as listed plus charges for any tests undergone by patient.
